Multi-dimensional approach has been shown to be effective in management of chronic pain. It usually involves a self management program along with combination of other interventions. Despite this success, a significant number of referrals fail to complete treatment or achieve improved clinical outcomes. One of the factors which may influence these outcomes is a patient's readiness for change. One of the means of defining the readiness of patients to engage with rehabilitation and self-management is through adaptation of the stages of change trans-theoretical model (Prochaska & DiClemente 1983).

Multi-disciplinary approach to chronic pain management has been considered the most effective approach in addressing the limitations, reducing pain and improving functional outcomes. However, most of the programmes simply patch together concurrent physiotherapy and psychology sessions without true integration of underlying philosophy of multi-disciplinary approach.

Ankle fractures are among the common traumatic injuries that present to hospitals which can result in significant functional impairment and disability. Rehabilitation Case Management in ankle fractures is focused around healing and recovery of functional capability to facilitate return to activities of daily living, leisure and work. In this article, I look at some of the considerations for fracture management and rehabilitation from a case manager's perspective.
